If you are willing to write such a letter, we ask that it be typed on official letterhead stationery and submitted directly along with this form to the Career Services Office indicated above. If you are not comfortable writing a letter of recommendation, we suggest that you tell the requester you are uncomfortable with his/her request and that he/she might want to ask someone else. A student's candidacy may be hurt with a lukewarm reference letter.
 
Under the Federal Law P.L. 93-380 (Family Educational Rights and Privacy Act of 1974), also known as the Buckley Amendment, students have a right to read letters of recommendation housed in their dossier. The bottom portion of this form indicates whether the student making this request retains or waives this right.
